Getting Medical Care in China as a Foreigner: How Travel Insurance Claims Actually Work外国人在中国看病:旅行险理赔到底怎么走
A practical guide for foreigners in China: choosing a hospital, paying upfront vs direct billing, which documents to collect, and why travel insurance claims get delayed.
Falling ill or getting hurt far from home is stressful enough. In China, the hospital system, the paperwork and the language can make it feel even harder. This guide explains, in plain terms, how medical care usually works for foreigners here and what a travel insurance claim actually involves, so you know what to do before, during and after a hospital visit.
1. International hospitals vs public hospitals1.
Broadly, you have two options. Private international hospitals and clinics, found mainly in larger cities, usually have English-speaking staff, shorter waits and an experience similar to what many foreigners know from home. They also tend to cost considerably more.
Public hospitals handle the vast majority of patients in China and include many highly respected teaching hospitals. Many large public hospitals have an International Medical Department or a VIP / special-needs department (often called 国际部 or 特需部) that offers appointments, some English support and a quieter environment, at a higher price than the general outpatient department. In the general departments you will typically register at a counter or through an app, pay before each test or prescription, and move between several windows, which can be confusing without Chinese.
Which one is right depends on your situation and your policy. Some policies have limits that make expensive private hospitals hard to afford for longer stays, while others work smoothly with certain international providers. In a real emergency, go to the nearest suitable hospital first and deal with insurance afterwards.
2. Paying upfront vs direct billing2.
There are two basic ways an insurance claim gets paid. With reimbursement, you pay the hospital yourself, collect the documents, and submit a claim to the insurer afterwards. This is the most common situation for foreigners in China, especially at public hospitals and for outpatient visits.
With direct billing, the insurer (or its assistance company) pays the hospital directly. This is usually only possible at hospitals that already have an arrangement with the insurer, and often only for inpatient stays. It normally requires a guarantee of payment before or soon after admission, so contacting the insurer's assistance line early matters. Even with direct billing, you may still pay for items the policy does not cover, plus any deductible.
Scenario: A visitor developed a high fever and went to the general outpatient department of a public hospital. She paid at each step through a payment app and kept the screenshots, but did not ask for official invoices. When she filed her claim, the insurer asked for the invoices, and she had to return to the hospital to request them.
3. Documents to collect3.
Missing paperwork is one of the most common reasons claims slow down. Before you leave the hospital, try to collect:
- Official invoices (发票) for every payment. A payment app screenshot is usually not enough.
- Itemised fee lists (费用清单), especially for hospital stays.
- Medical records: outpatient record (门诊病历), or for inpatients the discharge summary (出院小结).
- A diagnosis certificate (诊断证明) with the hospital stamp.
- Test and imaging reports, and prescriptions.
- A copy of your passport, visa or residence permit, and proof of entry dates if asked.
- For accidents: a police or incident report where relevant.
- Translations, if your insurer requires documents in English or another language.
Keep the originals safe and make clear photos or scans of everything. Some insurers accept copies, while others ask for originals.
4. Typical timelines4.
Timelines vary by insurer and case, but the process usually runs in stages: notify the insurer (as soon as reasonably possible, and before planned treatment or admission where the policy requires it), gather documents, submit the claim, answer any follow-up questions, then receive a decision and payment. Policies usually set a deadline for submitting claims, so check yours and do not leave it until you return home. Simple outpatient claims with complete paperwork tend to move faster than hospital stays or cases where the insurer needs more medical information.
5. Common reasons claims are delayed or denied5.
- Missing or incomplete documents, especially invoices or medical records.
- The insurer was not told in advance about a planned admission when the policy required it.
- The condition began before the policy started, or falls within a waiting period.
- The treatment is excluded, for example routine check-ups or some dental care, depending on the policy.
- Name or date mismatches between passport, invoices and records.
- Documents the insurer cannot read, with no translation provided.
Scenario: A traveller was admitted after a minor accident and assumed his card would be accepted. The hospital had no arrangement with his insurer, so he paid the deposit himself. Because he called the assistance line on the first day and kept every document, his reimbursement claim went through without major issues.
Scenario: A long-stay visitor's claim was questioned because her records used a Chinese transliteration of her name that did not match her passport. A short letter from the hospital confirming her identity resolved it, but it added weeks.
6. How a licensed local agent can help6.
A licensed insurance agent based in China cannot change your policy terms or guarantee a claim outcome. That decision always belongs to the insurer. What a local agent can do is make the practical side easier:在中国执业的持牌保险代理人无法改变你的保单条款,也不能保证理赔结果,那始终由保险公司决定。
- Help you choose a suitable hospital or department and, where possible, accompany you.
- Explain what to ask for at the counter, so you leave with the right invoices and records.
- Help with Chinese-English communication and translation of documents.
- Check your claim file for gaps before you submit it.
- Explain policy terms in plain language before you buy, so there are fewer surprises later.
